Ideas for Hurt and Heal 4?

We want to make sure many people play this. I apologize for my selfish decision I made that made Hurt and Heal 3 the worst... I shouldn't of just added specials cuz I wanted them, they were cool, but not used affectively to be considered "good".

Now this time.. I want your say. Tell me what you want in Hurt and Heal 4! Give me a full out idea on how the playstyle would work. Thanks for reading
