I always do Nevada first for a major reason (not saying what because spoilers) and London second both because it houses one of my favorite levels ever in a Tomb Raider game (Aldwych ftw <3) and because of the general atmosphere of the London levels. I do the South Pacific levels last for a climatic showdown with ol' mama T-Rex and because I don't like the river rafting sections.
Usually, not too often. I like to reserve my grenade launcher ammo until the antartica levels where bullets become too weak to work, and I save my rockets for the final fight in the game.

For the first game, I go with ol' Gutsy, followed by Cutman, Elecman, and so on. For other games, it varies greatly, although in 2 I start out with Metal Man and in 3 I usually start with Top Man. I think MM1 is the first game I actually follow a set pattern to ahah
